IMPORTANT NOTICE TO PARENTS AND GUARDIANS

This form must be completed and signed by a parent or legal guardian before any member under the age of 18 may participate in live coaching sessions, submit videos for critique, or engage in any direct coaching interaction with Sophie Pittman. Please read this form carefully before signing.

• Grant permission for the above-named minor to participate in The Sparkles Method membership platform and coaching services offered by The Sophie Sparkles Method LLC
• Consent to the collection of the minor's name and email address as necessary to provide membership services
• Acknowledge that live coaching sessions are conducted via Zoom and consent to the minor's participation in these sessions
• Acknowledge that video submissions are private and visible only to Sophie Pittman
• Understand that Sophie Pittman is a professional dancer and coach, not a licensed physical therapist or medical professional
• Accept that participation in dance coaching activities carries inherent physical risk and I assume responsibility for my child's physical safety
• Agree to The Sparkles Method's Terms of Service, Privacy Policy, and all associated platform policies on behalf of the minor
